Writing Contest Entry Form
The Cottonwood Heights Arts Council, in cooperation with Salt Lake County’s Whitmore Library, is
sponsoring its 3rd Annual “Write for the Heights” writing contest for Salt Lake County residents aged 12
and older.
Entries must fit in one of these six categories: (1) Fiction by an adult, (2) Poetry by an adult, (3) Journalism
by an adult, (4) Fiction by youth (age 12-17), (5) Poetry by youth (age 12-17), or (6) Journalism by youth
(age 12-17).
Fiction must be less than 15,000 words long. Journalism entries should be less than 2,000 words. ONLY
[email protected], or submit paper copy entries at your local branch of the Salt Lake County Library.
Please include the author’s initials in the upper right hand corner of each page of your entry if submitting a
paper copy.
Sign the release agreement—this allows Cottonwood Heights city to publish your work on the city’s website,
should you win the contest. Attach this form to the submission, whether in paper form, or in e-mail.
Submissions without a signed Release Agreement (provided by the city) will not be published.

Entries must be submitted by May 11, 2013, 6:00 p.m. if submitting a paper copy at a County Library.
For e-mail submissions, the deadline is May 11, 2013, 11:59 p.m. (as determined by the recipient’s email
time stamp.) No late submissions will be accepted. A panel of librarians, educators, and local writers will
judge the entries.
There will be one grand-prize winner and two honorable mentions per category. Winning entries will be
announced in mid-June at a closing event. With a signed Release Agreement, winners’ work will be published
on the city’s website. If you do not sign and submit the release form, your work will not be published on-line.
Additional publication rights will remain with the author.
Release Agreement
FOR GOOD AND VALUABLE CONSIDERATION, the receipt and legal sufficiency
of which is hereby acknowledged, _____________________________an individual whose
address is ___________________________, ____________________, UT 84___ (“Artist”),
hereby grants to COTTONWOOD HEIGHTS, a municipality of the state of Utah whose
address is 1265 East Ft. Union Blvd., Suite 250, Cottonwood Heights, UT 84047 (“ City”), a
nonexclusive and irrevocable license (“License”) , worldwide and in perpetuity, to use the artistic
works (the “Content”) shown on the attached exhibitat any time through any medium of the
City’s choosing. Artist agrees and consents that the City may:
Use, re-use, publish and re-publish the Content in whole or in part, severally or in
conjunction with other artistic content, in any and all medium (including but not limited to any
traditional and/or social media venue) and for any other purpose whatsoever, including but not
limited to illustration, promotion, advertising, and trade;
Use Artist’s name in connection with the Content or any derivative works created
therefrom; and
Assign or transfer the rights described above, in whole or in part, to any third
party for use in any derivative or creative works prepared by or with the City for the benefit of
such third party.
Artist represents and warrants that: (a) Artist is the sole author and owner of the Content
and all intellectual property associated with the Content; and (b) the use of the Content and
associated intellectual property and other rights granted to the City under this Release Agreement
shall not infringe upon any copyright or any other intellectual property rights of any third party.
Artist hereby releases and discharges the City and its successors, licensees, assigns and
representatives from any and all claims and demands arising out of or in connection with the use
of the Content, including any and all claims for libel, infringement of copyright, invasion of
privacy, or infringement of any rights of publicity.
