Lessons for Success Principal Investigator Consent Form

___________________________________________ is applying to the American SpeechLanguage-Hearing Association’s Lessons for Success (LfS) program. One of the purposes of
this program is to provide intensive training and mentoring in research grant preparation and
funding opportunities. Additional information about the LfS program is available at

As part of their application package, applicants submit grants that will be discussed at the
workshop in small group mentoring sessions, as well as in mock and section reviews. Your
